Who qualifies
Adults 65 and older or adults 21 and older with a physical disability enrolled in TennCare. Members must meet nursing facility level of care criteria (Groups 1 and 2) or be at risk of institutional placement (Group 3) based on a Pre-Admission Evaluation.

Common questions about TennCare CHOICES
How do I know if I qualify for CHOICES?
CHOICES eligibility is determined by a Pre-Admission Evaluation (PAE) that measures functional need. Groups 1 and 2 require nursing facility level of care; Group 3 is for adults at risk of institutional placement. Your MCO care coordinator schedules the PAE.
Can I request Resource One as my provider?
Yes. TennCare members have the right to choose their HCBS provider from their MCO's approved list. Call your MCO care coordinator and request Resource One Medical Staffing by name.
What services does CHOICES actually cover?
Personal care (bathing, grooming, transfers, toileting), homemaker services (housekeeping, laundry, meal prep), attendant care, and in-home respite for family caregivers. Specific hours are set in your care plan.
What's the difference between Groups 1, 2, and 3?
Group 1: adults in a nursing facility transitioning home. Group 2: adults who meet nursing facility level of care and choose home-based services instead. Group 3: adults at risk of institutional placement who get preventive home-based support.
Can CHOICES be combined with other programs?
CHOICES can be combined with VA benefits for eligible veterans and the National Family Caregiver Support Program for eligible caregivers. It cannot be combined with OPTIONS (you're either in TennCare or you're in OPTIONS).
